Vitamin B12 deficiency in Pakistan: symptoms, causes, and what helps
B12 deficiency is remarkably common here — and not only among vegetarians. What it actually does to the body, who is most at risk, and the dietary changes that help.

Of all the deficiencies I check for in a first consultation, B12 is the one patients are most surprised by. Vitamin D, iron — those feel familiar. B12 rarely comes up until the lab report does, and by then it has usually been affecting someone's energy and mood for months.
How common this actually is here
A study comparing vegetarians and omnivores at a Karachi teaching hospital found vegetarians had significantly lower B12 levels and higher methylmalonic acid — a marker of functional deficiency — along with notably more depression, paraesthesia and peripheral neuropathy. But the same paper cites a separate Pakistani study finding B12 deficiency in 57 percent of people after excluding strict vegetarians entirely — meaning this is not purely a vegetarian-diet problem in this population.
What it actually does to the body
B12 is essential for nerve function and red blood cell production, which is why deficiency shows up as a cluster that looks like several unrelated problems at once: persistent fatigue, pins-and-needles in the hands or feet, poor concentration, low mood, and in more advanced cases, balance problems or memory changes. It is frequently mistaken for being simply tired or stressed, because nothing about it looks dramatic early on.
Who is actually at higher risk
- Vegetarians and anyone eating little meat, egg or dairy — B12 is found almost exclusively in animal foods.
- Anyone on long-term metformin or acid-reducing medication, both of which are known to reduce B12 absorption.
- People over 50, since stomach acid — needed to release B12 from food — naturally declines with age.
- Anyone with an undiagnosed digestive condition affecting absorption, regardless of how much B12 is in the diet.
What actually helps
If a blood test confirms deficiency, dietary change alone is usually too slow to correct it — this is one of the few cases where supplementation, sometimes by injection for more severe cases, genuinely outperforms food alone, at least to start. Once levels recover, maintaining them through diet is realistic for most people: eggs, dairy, chicken and fish are the most practical regular sources in a typical Pakistani kitchen.
“I see this most in patients who eat well by every obvious measure — good portions, regular meals — but have quietly cut back on meat, egg or dairy without replacing what those foods were providing.”
If fatigue has been a recurring theme regardless of how much you sleep, it may be worth ruling out iron too — see Iron Deficiency Anaemia: A Pakistan-Specific Diet Guide. For a plan built around your actual labs, book a consultation.
